By signing this form I give Artist on the Road permission to store my personal data, conform the privacy policy of Artist on the Road.

I give permission under the following conditions:

• I only give my permission for the data that I provide by filling in this form.
• Personal data will be treated confidentially en carefully.
• When it’s not, or no longer, necessary to store my personal data, Artist on the Road won’t register my data or will delete it.
• I can choose to not give my permission, or to only give permission for certain parts. Artist on the Road will explain the consequences if I, for any reason, won’t give my permission to store any personal data.
